The following forms are required prior to your pets stay: 

 

Service Agreement (signed and submitted annually):

- The service agreement is found HERE.  Please sign and submit the form in the window below the agreement.  A confirmation will be sent to your email upon submittal.  If technology is not cooperating, we also have hard copies available to sign when you drop off your dog.  

 

Information Sheet (valid as long the information remains the same):

- The information sheet is found HERE.  This form tells us more about you and your pet, and it provides necessary emergency contact and veterinary information.  Please submit the form when complete.  A confirmation message will appear upon completion of the form.

Vaccination Record (valid for 1 year, unless vaccine expires during this time, in which case an updated record will be required):

- Vaccination records must be provided by your vet.  Completed vaccination forms can be uploaded HERE, emailed to DogsofDorr@Gmail.com, or texted to (616)358-0030.

- Required  up to date vaccines are listed below:

• DPP (Distemper, Parvovirus, and Parainfluenza)

• Rabies

• Bordetella

• Must be flea and tick free.
